Combined hazard = exp(0.60 · Σ ln·HR): the 0.60 factor shrinks the naive product because these behaviours overlap (fit people also tend to be lean, active non-smokers). Biological-age delta uses a Gompertz mortality doubling of ~8 years and is capped to ±(14–20) yrs.
